Service Description

Your physician may refer you to receive an evaluation before insurance will approve your bariatric surgery. Your entire medical team wants to maximize your success to become a healthier person and improve the quality of your life to the greatest possible extent, we want to join your team! A test will help identify strengths, an understanding of the effects of surgery, or a supportive friend or family member. It can also help identify areas you might need help with after surgery, such as help with depression, unsupportive family or past patterns and causes of emotional eating.

Cancelation Policy

Failure to cancel within 24 hours will result in a cancellation fee or charge for the full cost of the service. To cancel, please send an email to contact@cruduppsychservices.com, with "cancel appointment" in the subject line. To reschedule, please book an appoint via our online portal. To be added to a wait list, include that information in your cancellation email.
